What documents should I attach?

If your account was registered in the name of an individual person, you will need to provide a color copy of that individual's Drivers License or State ID.

If the account had a company name listed during registration, you will need to submit a copy of one of the following documents as proof of ownership.

• Business License
• Certificate of Association
• 501(c)(3)
• DBA/ Fictitious Name Documentation

What information should be on the documents?

• Individual/Company Name
• Street Address matching the address on the account

A Request Type needs to be chosen

Request Type Select the one that applies

• Domain Ownership Verification
• Account Ownership Verification
• IRTP Ownership Verification

Username or Domain ^*

Invalid Username (should only contain Characters and/or Numbers)

Email Address ^*

Email must be entered and be in a valid email format

Attach Documents ^*

A File must be attached.

All files must be smaller than 25mb.

Files must be of type pdf, jpg, tiff, doc, docx, or png.

You can select multiple files in the Choose Files menu by holding Command (on Mac) or Control (on Windows) key when selecting. File size limit is 25mb. Allowed file types are pdf, jpg, tiff, doc, docx, or png.
